Case Management

We assist with coordination of services, finding needed community services, advocacy and outreach, individual and family education and training, and eliminating barriers to seeking or maintaining education and employment.

Our agency provides authorized representation/power of attorney services for the general community. Our primary focus is health and human services as well as supportive service agencies. We assist our clients by either going with clients or for them to social service agencies such as Welfare, Social Security, or HEAP.

We will:

  • Have access to eligibility information in applicant’s or enrollee’s file
  • Attend local agency interviews
  • Complete forms
  • Sign forms, such as renewals for the applicant or enrollee
  • Provide documentation
  • Appeal agency decisions
  • Receive forms & notices

Homemaker/Personal Care

Supports a person to be more independent while meeting their daily living needs. Direct service providers help people with household chores and personal care, including things like:

  • getting dressed and keeping clean
  • making food
  • managing money to pay household bills
  • cleaning and doing laundry
  • getting out and being a part of the community
  • helping people to get to medical and dental appointments or other health care services
  • assisting a person’s ability to express their opinions and choices.

Transportation

Offering people with disabilities a way to access their waiver services, community activities, and resources when other people or agencies cannot provide transportation free of charge.

Transportation services may be available based on an understanding of a person's needs discovered during an assessment, then listed in their individual service plan.

Whenever possible, family, friends, neighbors, or community agencies that provide transportation without charge should be used first.

Under the Level One Waiver, payment for Community Respite, Homemaker/Personal Care, informal respite, Money Management, Participant-Directed Homemaker/Personal Care, Remote Support, Residential Respite, and Transportation, alone or in combination, can not exceed $6,750 per waiver span.

Adult Day Support

We use the Ohio Administrative Code that follows the rule that defines adult day support and sets forth provider qualifications, requirements for service delivery and documentation of services, and payment standards for the service.

The expected outcome of adult day support is development of skills that lead to greater independence, community membership, relationship building, self-direction, and self-advocacy.

Definitions For the purposes of this rule, the following definitions apply:

1. "Adult day support" means provision of regularly scheduled activities in a non-residential setting, such as assistance with acquisition, retention, or improvement of self-help, socialization, and adaptive skills that enhance the individual's social development and performance of daily community living. Adult day support shall be designed to foster the acquisition of skills, build community membership and independence, and expand personal choice. Adult day support enables the individual to attain and maintain his or her maximum potential. Activities that constitute adult day support include but are not limited to:

  • Supports to participate in community activities and build community membership consistent with the individual's interests, preferences, goals, and outcomes.
  • Supports to develop and maintain a meaningful social life, including social skill development which offers opportunities for personal growth, independence, and natural supports through community involvement, participation, and relationships.
  • Supports and opportunities that increase problem-solving skills to maximize an individual's ability to participate in integrated community activities independently or with natural supports.
  • Personal care including supports and supervision in the areas of personal hygiene, eating, communication, mobility, toileting, and dressing to ensure an individual's ability to experience and participate in community living.
  • Skill reinforcement including the implementation of behavioral support strategies, assistance in the use of communication and mobility devices, and other activities that reinforce skills learned by the individual that are necessary to ensure his or her initial and continued participation in community life.
  • Training in self-determination which includes assisting the individual to develop self-advocacy skills; to exercise his or her civil rights; to exercise control and responsibility over the services he or she receives; and to acquire skills that enable him or her to become more independent, productive, and integrated within the community.
  • Recreation and leisure including supports identified in the person-centered individual service plan as being therapeutic in nature, rather than merely providing a diversion, and/or as being necessary to assist the individual to develop and/or maintain social relationships and family contacts.
  • Assisting the individual with self-medication or provision of medication administration for prescribed medication and assisting the individual with or performing health-related activities in accordance with Chapters 5123-6 and 5123:2-6 of the Administrative Code.
Shared Living

"Shared living" means individual-specific personal care and support necessary to meet the day-to-day needs of an adult enrolled in the individual options waiver, by an adult caregiver who resides in the same home as the individual receiving the services.

Specific personal care and support necessary to meet the day-to-day needs of an adult enrolled in the individual options waiver, by an adult caregiver who resides in the same home as the individual receiving the services. Shared living is provided in conjunction with residing in the home and is part of the rhythm of life that naturally occurs when people live together in the same home. Due to the environment provided by living together in the same home, segregating these activities into discrete services is impractical.

Money Management

Through this service, a direct service provider can help a person with managing their personal finances, like writing checks, paying bills, and keeping track of cash, income, or benefits. The service also includes training to assist a person to acquire, retain, or improve those skills.

Money management can include:

  • bill-paying and preparing checks
  • balancing checkbooks, reconciling bank account statements, and maintaining or organizing bank records
  • preparing and delivering bank account deposits
  • assisting the person with applying for benefits
  • assisting the person with maintaining eligibility for benefits
  • consulting or making referrals for consultation regarding available benefits
  • making referrals as appropriate for establishment of disability accounts
  • organizing tax documents and other paperwork
  • negotiating with creditors
  • deciphering medical insurance papers and verifying proper processing of claims
  • providing general organization assistance
  • providing referrals to legal, tax, and investment professionals
  • notarizing documents
  • providing assistance associated with financial tasks when a person relocates
  • acting as power-of-attorney or authorized representative when designated by the person
  • being the appointed payee
Residential Respite

Respite care provides short-term relief for primary caregivers. It can be arranged for just an afternoon or for several days or weeks. Care can be provided at home, our adult day center.

The person’s service and support administrator, or SSA, will assess them for a need for Residential Respite. If there is a need, the SSA will include it in the person’s individual service plan.

Residential Respite is limited to 90 calendar days per waiver span.
